In addition to the CSU Graduate School online application, the department requires applicants to add the following forms and statements to their application package.

• Letter of Recommendation Form (PDF) (MS Word) (required for all applicants)
• Personal Interest Statement (required for all applicants)
• Foreign Student Financial Support Statement (PDF) (required only for applicants outside of the U.S.)
